- After submitting we will send you the "Credit Card Authorization Form" via Docusign to fill out and e-sign.
- *Important: Your information will be kept secure and used for identity verification purposes. This will aid us in the prevention of unauthorized purchases or other forms of fraud.
- Once the Docusign form is completed/submitted you may cancel the authorization at any time by contacting us. This authorization will remain in effect until cancelled.
- You may choose to allow us to hold your information for verification purposes for up to 30 days OR save your information for future purchases
